For case managers, care managers and agencies

Your caseload in one place, with families kept informed.

Unique Care Hub gives every person under your coordination one record, and every family a clear window into it. Family visibility is configured record by record, and the day is recorded on a phone by whoever is present — so fewer updates have to come through you.

Thirty minutes, based on a real caseload. No slide deck.

What changes for you

Four things that change day to day.

One caseload view

Everyone on the caseload, what is still unrecorded today, and what is overdue — on one screen in the morning.

You configure the family view

Per record, per family member, per field. Mood, appetite, notes and documents can each be shown or hidden, so two families on the same caseload can be set up quite differently.

A record you can rely on

Medication changes keep their history rather than being overwritten, each entry is saved with the name of the person who recorded it and the time, and archiving takes the place of deleting throughout.

Escalation on your terms

Scheduled, then due, then unrecorded, then flagged — with the timings set for each record. A late lunch is not an incident, and is not treated as one.

Scope

Designed for care coordination.

Unique Care Hub complements — but does not replace — medical records, compliance systems, professional advice, or emergency care. The categories below carry real regulatory weight, and the product does not cover them.

  • Not an electronic medication administration record, and not at this version.
  • No electronic visit verification. Our check-in is a handover between caregivers, not a compliance record.
  • No Medicaid or insurance billing.
  • No payroll, and no scheduling engine for an entire workforce.

The focus is narrower: one record per person that the family can genuinely read, and a day that reliably gets written down.

Where we are

A working platform, entering supervised pilot testing.

All three interfaces — family, caregiver and care manager — are built, connected to one shared record, and exercised daily against realistic care situations.

Onboarding is supervised and by invitation, one household at a time, so that every account is opened deliberately and traceably. The terms of service, privacy policy and authorization notice are published in the hub and shown for acceptance before anyone reaches a record; they are drafts pending review by an attorney, and they say so on their face. No certification, external audit or compliance approval is claimed today. Anything that is completed will be published in plain terms and dated.

Pricing is not settled. It will be a subscription per active person per month, and the figure is being decided together with the first participating organizations rather than published untested. Nothing is charged during the pilot, and the platform takes no payment details.

Working session

Bring one real situation and the team will set it up with you.

Thirty minutes, using fictional details that stand in for a real situation from your caseload. Onboarding is by invitation and supervised, so every organization gets a proper walkthrough rather than a login and a link.

This site collects nothing. There is no form here and no tracking. Write to the address below and a founder replies. Please don't include names, diagnoses or anything else about a person's health in a first email.
